It is interesting that the vast majority of the time, a claim was changed from pended status to denied, because the Medicare contractor never received the requested documentation.
So, be prepared by first implementing good documentation processes, and then when the medical record is requested—send it via certified mail, return receipt requested so that you have proof that it was received.
